I just noticed that the lower right lug is flush with the bracelet and the others extend past the end links. Do you think there was a lot of material removed from those 2 lugs?

Doubt very much it was done by Rolex either; it's way off "original condition" and in my view warrants a new case.

If it was done by Rolex, the guy should be fired. It wasn't something that took me a long time to notice, as soon as I saw the picture it didn't look right.
